Do not enter a team name or jersey number.
Player's name must be entered as it appears on the birth
certificate. No nicknames. A middle initial is
required unless player does not have one.
Parent must sign in presence of a notary.
Once form is completed and notarized, parent must make two
(2) additional copies so that 3 total copies are submitted
at tryouts.
If using the triplicate version, please ask the notary to stamp
all three pages.
